your exam, the doctor will review her findings, discuss alternative
treatment choices, and formulate a treatment plan specifically for your
needs and values. Your input is very important! Our goal is to give you a
clear understanding of your dental health so you can make a truly informed
decision.
Your treatment plan will include a breakdown of all applicable fees, and
we will inform you of all costs before treatment is administered. Charges
are payable at the time of treatment. For your convenience, we offer a
range of payment options to meet your needs.

and Discover. Our practice accepts personal and certified checks,
but not business checks.

CITI HEALTH CARD / CARE CREDIT
We
also offer financing through CITI Health card or Care Credit to make dental care, general or
cosmetic, affordable for all patients who desire it. Healthcard is available
for all procedures ranging from $300 - $25,000, including: Cosmetic /
Aesthetic Dentistry, Periodontics, Prosthodontics, Endodontics and Oral
Surgery. The benefits of the plan include:
- No initial payment
- Low, fixed rates
- Low monthly payments
- First Payment Not Due for 4-6 weeks

MEDICAL SPENDING ACCOUNT
Your company may offer a medical spending account, which allows you to set
aside pre-tax dollars to pay for your medical expenses. Please contact
your employer for more details.

COVERAGE
Our practice will be happy to assist you in determining whether your
insurance company will cover your Dental care. Additionally, we will submit all insurance claims for you. We will
